Paul Farmer, MD, PhD, Chief, Division of Global Health Equity at Brigham and Women’s Hospital (BWH), discusses how BWH physicians are working with partners around the world to strengthen health systems and reduce health disparities. Dr. Farmer notes that there are three components of a health system that are essential for delivering care effectively: hospitals to care for the critically ill or injured, outpatient clinics in convenient locations and community health workers who can deliver care to those in remote areas. Dr. Farmer cites the example of Rwanda, which has 60,000 community health workers for a population of approximately ten million people. Rwanda outperforms the US in terms of health outcomes for people with chronic illnesses such as AIDS or tuberculosis. The impact of community health workers in Rwanda has been significant: the steepest declines in mortality ever documented anywhere at any time have been in Rwanda over the last decade.
