Prevention, early detection and management of chronic kidney disease (CKD) are key to supporting patients at the primary care clinic level. In this six-session training, you will learn techniques to work collaboratively across the health care continuum to manage CKD and proper management in the transition from CKD to end-stage renal disease (ESRD)/dialysis. Health equity is addressed in each session. Shared resources will provide knowledge and structure of strategies to take back to the clinical team validate and improve current processes that focus on care for patients who have, are predisposed to, or are at risk of chronic disease.
