In this video, Dr Jay Mehta explains to viewers all the steps of the Obstructive Azoospermia treatment procedure and gives them a detailed understanding of the entire procedure. Azoospermia is a medical condition leading to Male Infertility which signifies ZERO Sperm Counts or Nil Sperm Counts. There are 2 predominant types of Azoospermia Obstructive Azoospermia: In this, the male has a good Sperm Sperm production but due to blockage of the male reproductive tract, there is a complete absence of sperm in the semen. Non Obstructive Azoospermia: In this, there is not enough sperm production to be seen in the semen. due to a variety of genetic, endocrinological, lifestyle and medical conditions. This condition is more common than Obstructive Azoospermia. Obstructive Azoospermia Treatment The treatment choice for Obstructive Azoospermia is very straightforward because the reports indicate that the sperm production is going to be normal inside the testis, what is important here is to observe that the testicular volume is going to be normal. In this process, sperm retrieval requires Testicular Fine Needle Aspiration called TEFNA. Once the TEFNA is done the sperm recovery rate following Obstructive Azoospermia is going to be high at 95%. These sperms are obtained once the testicular tissues are gifted to the laboratory where it undergoes further processing. These sperms are further processed to perform ICSI (Intracytoplasmic sperm injection) wherein the manual fusion of immotile sperm is done with the oocyte. Why ICSI following TEFNA for Obstructive Azoospermia? This is because of the motility as a result of which doctors instead of standard in vitro fertilization (IVF) go for ICSI it has shown to result in a significantly higher fertilization rate. On average, the clinical pregnancy following Testicular Sperm Extraction for Obstructive Azoospermia is almost equivalent to the clinical pregnancy rate of any other pregnancy following IVF. This process has slightly better LIVE BIRTH rates against Non Obstructive Azoospermia Non Obstructive Azoospermia (NOA) is a very challenging condition to treat. In the hands of a good Fertility Specialist, one can expect a Sperm Retrieval Rate (SRR) of 55-65%. The most technically sound surgery for the same is a micro TESE, a prolonged surgery which needs tremendous patience and concentration to scan through the entire testis in order to retrieve sperms which are healthy enough to obtain healthy Blastocyst Embryos. Though it’s a process which is dependant on variable factors, the estimates success rates of LIVE BIRTH following micro TESE for Sperm Extraction are close to 25-30%. A lot of the couples with Azoospermia can facilitate to have a Genetic Child of their Own, thanks to such advanced techniques. International travel is allowed 48 hours post-procedure Now, typically, this process is carried out along with the process of Ovarian Stimulation in order to obtain the sperms on the day of the oocyte retrieval. This is done to save time and reduce the financial burden on the patients.
