Microsclerotherapy is currently the best treatment for leg thread veins. Dr Haroun Gajraj of The VeinCare Centre explains the procedure in detail and shows you a video of the actual medical procedure. Warning - viewer discretion is advised. Thread veins (also known as spider veins or telangiectasia) are best treated by injecting a prescription medicine (POM) called a sclerosant. In the United Kingdom, the only licensed product for the treatment of leg thread veins, reticular veins and telangiectasias is 0.2% STD (sodium tetradecyl sulphate or Fibrovein 0.2%). The sclerosant works by removing the lining of the vein almost instantly (this is not painful) and thereafter the vein responds with a healing process that leads to sclerosis and obliteration of the thread veins. The healing process may take several weeks to complete. Initially, the thread veins may look worse in the early stages of the inflammatory healing process. Often, 2 or 3 treatment sessions are required with an interval of 6 to 12 weeks. Ultimately, the thread veins become less visible and in some cases may disappear completely. After 3 treatment sessions, 90 to 95% of treated patients are "pleased" or "very pleased". Many vein specialists (myself included) recommend that graduated medical grade compression hosiery is worn after micro-sclerotherapy. Satisfaction levels after microsclerotherapy are high and serious complications are very rare. Commonly, the skin overlying the treated veins may become pigmented. This brown discolouration is the "ghost outline" of the successfully treated thread vein underneath. My opinion is that the pigmentation is a natural reaction of the melanocytes to the inflammation (post-inflammatory pigmentation). The discolouration is largely faded by 6 months but very occasionally may take up to 2 years to disappear completely. Serious allergic reactions, neurological complications or deep vein thrombosis are extremely rare indeed. In my opinion microsclerotherapy should only be performed by suitably trained nurses or doctors and only after a full assessment by hand held Doppler or colour duplex ultrasound scan.
