Updated version of this video with radiograph and some annotations I was recently introduced to Kuraray's caries detector dye on a Biomimetic Dentistry course recently and it has quickly become one of those go-to materials that I wouldn't want to be without. I wish I had known about it years ago! The key in this case was to remove deep caries from the UL6 without exposing the pulp so we can give this tooth a chance of avoiding RCT. A perio probe is helpful for measuring your cavity depth and for making a decision about when to stop excavating, so combining measurements with the use of dye effectively gives us "guided" selective caries removal. I decided to keep the distal cusps of UL6 here even though they are somewhat undermined. I felt comfortable with this because we were able to get the remaining dentine so clean and then bond the tooth back together using David Alleman's Stress-Reduced Direct Composite technique to manage polymerisation shrinkage, but some dentists would prefer to reduce these cusps and restore with an onlay, which would also be a great option.
