Genomics in Medical Specialties - Gastroenterology

Genomics is increasingly important in every area of medicine. In this film, consultant gastroenterologist Dr Kevin Monahan explains the increasing relevance of genomics to gastroenterologists. Using examples from his practice and research, he shows that genomics is important for all gastroenterologists and emphasises the need for increased awareness and testing. Dr Monahan explains that all gastroenterologists in England now have access to genomic testing. He outlines how making a genetic diagnosis in an individual can have a number of benefits, both to the individual themselves in terms of clinical management and treatment, and to the wellbeing of others in their family. Dr Monahan looks at the importance of genomics by exploring a number of examples: People with Lynch syndrome should be identified so that they can be monitored closely using colonoscopy to prevent or catch cancer - to which they are predisposed - early. People with Lynch syndrome may be offered different treatments if they do develop cancer. As well as inherited conditions, genomics is important in common diseases such as coeliac disease and inflammatory bowel disease, too. Genomic testing can already be used for some common conditions, for example the HLA typing of people with suspected coeliac disease. Genomics can also play a role in avoiding adverse reactions to drugs, and some tests are available to determine whether someone is likely to have a bad reaction to a drug, or what dose might be appropriate for an individual. For example, testing for variants in the TPMT gene are conducted when considering the use of Azathioprine. Dr Monahan concludes by outlining his hopes for the future: that new knowledge about genomics can be translated into new therapies for patients as quickly as possible.
