I'm sure there is an easier, more effeciant way to do this, but this is a process that works for me. The better quality you have (aka put the card, paper, whatever and a flatbed scanner is better than just a picture of it with a cell phone, and that's better than pulling a low resolution picture off the internet) the better it will turn out. I use a very very very old version of photoshop, but the options I use should be in newer versions as well.
