A brief overview of the Houston Cities Changing Diabetes program, 2023. Cities Changing Diabetes - Houston supports people living with diabetes and obesity with resources and education, health navigation, and access to community and clinical programs. An extensive planning process involving a coalition of over 600 community stakeholders led by Houston Health Department has resulted in four key program areas: The Faith & Diabetes Initiative - an evidence-based Diabetes Self-Management Education and Supports programs for communities of faith to strengthen their health ministry's impact in the community. Houston Diabetes Peer Support Network - weekly meetings to create strong connections between people living with diabetes towards better self-management. Bite of HOPE - culinary education in Alief and Aldine communities for students, families, and community members. MyDiabetesHQ.org - a website featuring a community-building and navigation platform to support people living with diabetes on their journey to better health.
