Why does one have ribcage pain in ankylosing spondylitis? Ankylosing spondylitis causes inflammation in the joints that ribs make with the vertebrae. There is movement at these joints during breathing, coughing, sneezing. Ankylosing spondylitis also causes enthesitis. This refers to inflammation at sites where muscles attach to bones. Various muscles attach to the ribs & these sites get swollen. Inflammation of the said joints & enthesitis of rib muscles leads to the ribcage pain.
