The allergen which we found to be allergic in a patient whom we did allergic rhinitis are known as the hypersensitive reactions or the allergen are known as hypersensitised allergens. Desensitization is reversing those hypersensitisation. Desensitization or allergen specific immunotherapy is a more targeted treatment which specifically reverses the hypersensitivity reactions of that particular allergen. Now the specific indications for this allergen immunotherapy are moderate- severe allergic rhinitis or the persistent rhinitis or in the cases of a mild form of asthma. There are different modalities in which the treatment can be give, that is sublingual, where it can be given in the form of a drops under the tongue, or in an injectable form, that is the subcutaneous immunotherapy. What we do in desensitization therapy is we give incremental doses for which the patient is sensitized and is given over a period of time and the initial phase is the buildup phase and when it reaches the maintenance phase, it is given over a period of 3 years. Immunotherapy can be started from the age of 5 years, but with the advent of sublingual immunotherapy, they can start at the age of 3 years also. Allergen specific immunotherapy has a specific pharmacoeconomic advantages over the other modalities of treatment and it is the treatment which can prevent the progression of asthma and also the neosensitisation.
