Does creatine cause balding in men? This is a very commonly discussed "creatine side effect" that many guys ask me about. Although not fully understood, male pattern baldness seems to be very closely linked to an individual's sensitivity to DHT (dihydrotestosterone) which cuts off nutrients from the hair follicles. Creatine and hair loss has been a hotly debated topic because of a 2009 creatine DHT study on rugby players which showed a 56% increase in DHT levels after 1 week of creatine loading (25 grams per day), followed by an overall net 40% increase after the dosage was reduced to 5 grams per day over the last 2 weeks. The findings are definitely interesting and worth taking into account, but before throwing your creatine monohydrate in the trash, also take these points into consideration... Keep in mind that everything related to creatine and baldness comes from this one single study. The results have never been replicated and more research is definitely needed. DHT levels continued to drop after the loading phase ended, and since the study was only 3 weeks long we don't know what those levels would have looked like over a longer period. Creatine loading is not necessary, and it's possible that DHT levels may not have risen as high if a smaller dose was used right from the start. Hormone levels go up and down pretty sharply over the course of 24 hours, and we don't know if DHT was measured at the same time every day. Intense training itself causes an increase in DHT, and we have no information about the training programs used or how this may have played a role. Yes, you will find many forum threads from guys saying that creatine and hair loss are definitely connected since they experienced it firsthand, but I would only take those with a grain of salt. Millions of people supplement with creatine and millions also experience hair loss, and some people may accidentally draw a connection there where there is none. So, does creatine cause baldness? Ultimately we don't know for sure and you'll just have to take the available evidence into account and decide for yourself. Also keep in mind that creatine and male pattern baldness would only be an issue in those genetically pre-disposed to hair loss, since a rise in DHT is not the concern, but rather it's how your body handles that DHT. If you currently have a strong hairline and no signs of baldness, and hair loss doesn't run in your family, then I really wouldn't worry about it. Lastly, if you are prone to hair loss and want to be as cautious as possible, then just use a smaller 3 gram per day creatine dose as even this will allow for full benefits in most people.
