In this video we will discuss the Treatment of Asthma (asthma ka ilaj), drugs used in bronchial asthma, their side effects, and how to reduce their side effects. Goals of Asthma treatment (asthma ka ilaaj) 1) To control asthma symptoms (asthma ke lakshan) 2) To maintain normal activity of asthma patient 3) To maintain normal lung function 4) To prevent asthma attacks 5) To reduce the side effects of drugs used in bronchial asthma 6) To prevent any death from an asthma attack Drugs used in asthma treatment are of two types - 1) Quick Relief medicines - to control acute asthma attacks 2) Long term control medicines - to prevent asthma symptoms (asthma ke lakshan) Quick-relief drugs used in the treatment of asthma (asthma ka ilaj) belong to three classes First-class - Short-acting B2 agonist - Example -Albuterol, Levalbuterol. They relax the smooth muscles of the bronchi and dilate them Side effects - Tremors, Nausea, Fever, Vomiting, Headache, Dizziness Second Class - Anticholinergics- Example -Tiotropium, Ipratropium They block M3 receptors on smooth muscles of bronchi which results in their dilatation. Side effects -Headache, Flulike symptoms, dryness, Nausea, Dizziness, Constipation. Third Class-Corticosteroids -Example -Prednisone, Methylprednisolone, Prednisone. They reduce the inflammation in the airways. Side effects - Irregular heart rate, Hypertension, Disturb sugar control in the body, weight gain. Long Term Medicines - First Class - Long-acting B2 agonist Example Salmeterol, Formoterol. They relax the bronchioles of smooth muscles and relax the airways. Side effects - Headache, Sleep disturbance, Disturb sugar control in the body. Second Class - Inhaled Corticosteroids -Example - Budesonide, Mometsaone, Fluticasone Third Class - Non-Selective Phosphodiesterase enzyme inhibitors -Example -Theophylline Fourth Class -Mast cell stabilizers, Example -Cromolyn Sodium, they block the release of histamine in the body. Fifth class - Leukotriene receptor antagonist Example Zafirlukast, Montelukast Sixth Class - Monoclonal Antibodies - Omalizumab, Mepolizumab,Reslizumab,Benralizumab.They reduce the immune response of the body. Treatment of Asthma (asthma ka ilaj) is done according to the severity 1) Intermittent Asthma - No need of Long term controller medicines, only short acting b2 agonists are prescribed. 2) Mild Persistent asthma - Low dose corticosteroid or mast cell stabilizer or leukotriene receptor antagonist or theophylline can be given 3) Moderate persistent asthma - Low dose inhaled corticosteroids with long-acting B 2 agonist or medium dose of inhaled corticosteroids or low dose inhaled corticosteroid with leukotriene receptor antagonist 4) Severe persistent Asthma - High dose inhaled corticosteroid with long-acting B2 agonist or theophylline can be combined with long-acting oral steroids Other than medications patients should avoid exposure to environmental irritants like tobacco smoke, chemical smoke, dust mites, pollens. Bronchial thermoplasty - New technique for intractable asthma that does not show relief even after high dose inhaled steroids. It delivers thermal energy in a controlled manner in the airway wall.
