Цель игры — найти и спасти всех соседей, прежде чем до них доберутся зомби и прочая нечисть. После того как последний сосед будет спасён, прямо из воздуха материализуется дверь с выходом на следующий уровень. Если какой-нибудь сосед будет убит монстрами, то в следующем уровне соседей будет на одного меньше. При успешном спасении всех жертв в течение нескольких последующих уровней, их число вновь увеличится. Если игрок не успел никого спасти, то экран заливает кровью и игра заканчивается. Всего в игре 55 уровней (48 основных и 7 бонусных). Оружие и предметы выбираются и используются игроком только по его усмотрению (исключение — гамбургер, что сразу же восстанавливает часть здоровья). Радар (в версии для SNES может отключаться) показывает положение ближайших живых соседей. Мародерство — одно из основных занятий в игре, наравне с истреблением нечисти. Обшаривать брошенные дома и квартиры, рыться в чужих шкафах, ящиках и тумбочках жизненно необходимо для получения полезных вещей и нового оружия. Также, по мере продвижения, игроку придётся рыться ещё и в мусорных баках, амфорах, сундуках и кучах песка. Помимо обычного перемещения по локациям, игроку представлена возможность и для других способов передвижения: Батут — прыжки на батуте нужны для преодоления преград; Водоем — плавание под водой делает невозможным использование предметов или оружия. Главными героями игры являются подростки Зик и Джулия. Хотя нигде в игре имена героев не упоминаются, в продолжении Ghoul Patrol (вышедшем только на Super Nintendo в 1994 году) присутствуют и сюжет, и имена персонажей, перекочевавших из оригинальных Zombies Ate My Neighbors. The goal of the game is to find and save all the neighbors before the zombies and other evil spirits get to them. After the last neighbor is saved, a door with an exit to the next level will materialize right out of the air. If any neighbor is killed by monsters, then there will be one less neighbor in the next level. If you successfully save all the victims in the next few levels, their number will increase again. If the player does not have time to save anyone, then the screen is flooded with blood and the game ends. In total, the game has 55 levels (48 main and 7 bonus). Also, as you progress, the player will have to rummage through garbage cans, amphorae, chests and piles of sand. In addition to the usual movement through the locations, the player is presented with the opportunity for other ways of movement: Trampoline-trampoline jumping is necessary to overcome obstacles; Pond-swimming under water makes it impossible to use objects or weapons. The main characters of the game are teenagers Zeke and Julia. Although the names of the characters are not mentioned anywhere in the game, the sequel Ghoul Patrol (released only on Super Nintendo in 1994) contains both the plot and the names of characters who migrated from the original Zombies Ate My Neighbors. There is no difference between Zeke and Julia, except for their names, appearance, and gender. The antihero in the game is a certain Doctor Tong-an elderly bespectacled man in a white coat, who from time to time comes across the heroes during the game. The doctor drinks various potions all the time, the result of which is most often his teleportation to the Unknown (sometimes, after that, a flask with a red drink remains on the floor), but sometimes the doctor mutates into quite dangerous life forms. There are suspicions that he is to blame for the general chaos and for the fact that movie monsters have flooded the world.
