As torque is applied, the washer is pushing against the two opposing surfaces which creates enough tension to keep the bolt from backing out under usual circumstances. The flat washer has what I call a top and bottom side. The bottom side is the flat dull surface and the top side is the shiny convex (domed) side. The flat bottom should face the work piece and not the bolt head.
