Causes, prognosis and various techniques which are designed to fix frozen shoulder (adhesive capsulitis) are disclosed in this video as well as the true cause, prognosis, risks and complications. Current medical treatment options for frozen shoulder include arthroscopic frozen shoulder surgery and manipulation under anesthesia (MUA). Both are discussed in detail in this video. A medical procedure called "hydrodilation" is also available which involves heavily stretching the capsule of the joint by forcing an injected mixture of saline solution, local anaesthetic and steroid into the shoulder joint so as to create enough internal pressure that some of the scars and adhesions burst and rip open. A radiologist performs the arthroscopic procedure in the X-ray department using X-ray guidance to ensure the injection is accurately placed. Hydrodilation does not work at all in 30% of cases. For the remaining 70%, there is some evidence that arthrographic distension with hydrodilation provides short-term benefits in pain, range of movement and function in Frozen Shoulder but it is stated to be uncertain whether this is better than standard alternative interventions. The procedure is invasive and it is relatively common knowledge that prolonged X-ray radiation is not particularly healthy. Possible complications include joint infection, internal bleeding and damage to the joint capsule. Standard therapies including physiotherapy, chiropractic, osteopathy, active release technique (ART) do not immediately fix frozen shoulder and provide only very slow incremental improvements over many treatments at best. Usually these treatments are very painful and sometimes cause a worsening of the condition. Acupunture does not fix frozen shoulder although it may help in impingement syndrome. One of the most well known techniques, the Niel-Asher Technique involves repeated manual treatments over time. Although popular, it still only claims to get results after 10 treatments with months of care still usually being required. It is well marketed. A popular youtube video entitled Frozen Shoulder -- "The True Cause", has been put up by a chiropractor who claims that frozen shoulder or "frozen shoulder syndrome" is due to a dysfunction of the upper trapezius muscle and that it can effectively be fixed with "manipulation of the articulation between the occiput and the first cervical vertebrae". In other words, this chiropractor claims that frozen shoulder can be fixed with a chiropractic "adjustment" to the upper neck. The intention of this technique is apparently to stimulate the spinal accessory nerve so that the upper trapezius muscle pulls up the scapula in order for the humerus to be able to glide under the acromion allowing for free motion of the arm. Although this procedure may work well in cases of "shoulder impingement syndrome" it is a complete mystery how shoulder joint adhesions and scarring could be removed from an adhesive capsulitis with a chiropractic neck adjustment? Incredulous to say the least! The technique is called OTZ.
