USMLE Step 1 is Going Pass/Fail – What Does this Mean for Medical Students?

This news was just announced today by the FSMB, NBME and USMLE. There were a lot of licensing organizations involved in this change, but the bottom line is the impact on medical students is going to be huge. I’m going to dig into some initial thoughts in this video. Before we dive into the pass/fail change, let’s look at the other two major policy changes. The first is that the number of attempts on any USMLE exam is going to be limited to 4. Previously it was 6. To me, this doesn’t really affect most US medical students. The pass rate is well over 90% for US medical students, so almost everyone is passing on the first try anyway. The second change is that you must pass Step 1 before taking Step 2 CS. This makes sense, since you have to have the basic science knowledge before going to do the clinical skills assessment. Again, this doesn’t really impact the vast majority of US medical students at all. The third change does! Changing USMLE Step 1 from a three-digit score to P/F is going to have huge implications. Step 1 has always been the thing that strikes fear into medical students. Students are looking for high scores because it’s the test that residency program directors use to sort candidates and make decisions on who to interview. For years, students have used all the material they can, from Goljan to Pathoma, USMLEWorld, USMLERx, First Aid, it goes on and on and on. Med students hole themselves up for weeks to study for this test in order to get the best score they can. With scores out the window and it being pass/fail, all that stress suddenly melts away, for Step 1 at least. The goal of the USMLE score change to pass/fail was to spark systemic change and improve the overall transition to residency for medical students according to Michael Barone, VP of Licensure at NBME, and Dave Johnson, the Senior VP of the Federation of State of Medical Boards. Improving the transition from medical school to residency was an important consideration of these policy changes. But what happens when a candidate applies for residency though? Well, now that pressure of Step 1 is going to translate to USMLE Step 2 CK. Programs have to have a way of sorting through candidates. It’s like the SAT or MCAT. They just have too many candidates for too few spots. So for the most competitive specialties, Step 2 CK becomes an even bigger deal than ever. And for students, it’s enormous pressure as they now only have 1 shot to get a great grade, whereas previously if you didn’t do well on Step 1, you could make up for it on Step 2. Step 2 CK will now become the be-all and end-all. So, will the change really improve the stress levels of medical students? Maybe, but probably not. In fact, it may actually heighten it! One good thing is that Step 2 CK is a more clinically oriented exam, so it does correlate better to a physician’s performance than Step 1. Now residencies, of course, consider other factors. Research, letters of recommendation and clinical grades are all really important. Research, in particular, will play an even bigger role, as it’s another quantifiable measure (because ERAS allows PDs to sort by the number of publications a student has). The dean’s letter from a school will also play a bigger impact, as it often ranks medical students amongst their classmates with language such as “excellent candidate” or “very good candidate”. So, what’s the upshot? In January 2020 when this change takes place, medical students will have to take Step 2 CK well before the match and get a great score.
