Many patients visit us and they ask if any surgery or chemotherapy is needed or not. We tell them that we need to consider various factors before planning treatment. For example, we need to know what grade of the cancer is, how aggressive it is, what stage of cancer it is, and what part of it has already spread, whether it is the colon, the inner layer of the colon, the outer layer of the colon or spreads to another organ or lymph node. 1:18 Which tests are done to diagnose colon cancer? Biopsy, colonoscopy, CT scan, and PET scan are a few tests that are necessary to do before any treatment. They help us diagnose the stage of colon cancer. 1:34 What are the stages of colon cancer? There are 4 stages of colon cancer. Stage 1 is where colon cancer is confined to the mucosa or has spread to the muscles. Stage 2 is when colon cancer is confined to the colon. Stage 3 is when colon cancer has spread to organs or lymph nodes. And stage 4 is when colon cancer has spread to organs such as the lungs and liver. 2:21 How to classify the stage and treat it accordingly? First, we see which patient has which stage of colon cancer and then treat them accordingly. Stage 1 cancer is diagnosed easily. It can be removed by Endoscopy Treatment. In India, stage 2 and 3 colon cancer patients are found easily. So, they are treated with surgery followed by Chemotherapy (if necessary). 3:12 What are the types of colon surgery? Basically, there are 2-3 types of surgery. They depend on where the colon cancer is situated in the colon. If the tumor is on the right side of the colon, then the surgery is called Partial colectomy. When the tumor is situated on the left side of the colon, then it is called a Left Hemicolectomy. When the tumor is situated in the Sigmoid colon, then it is called a Sigmoid colectomy. We also find some cases where there are many tumors along the colon, then we suggest removing the Large Intestine. This is called Total colectomy. 4:30 How cancer surgeries are performed? During cancer surgery, we have to remove the tumor and the healthy part surrounding the cancer as well. For colon cancer, the margin is 10 cm. Blood vessels and lymph nodes are also removed. If the cancer is in stage 2 or 3 then surgery is the first solution in most cases. 5:30 What are the types of surgery? Previously, Open Surgery was done on Stage2 and 3 patients. Now, due to advanced technology, Minimally Invasive Surgeries are done. There are two types of Minimally Invasive Surgery. The first is Laparoscopy and the second is Robotic. If we compare the safety rate of recovery, it is the same for both surgeries. 7:02 What are the criteria for Chemotherapy (after surgery)? If the tumor has entered Stage 3 called T3 or T4. The tumor has passed all the layers of the colon or the cancer has reached a few lymph nodes. If the patient presents bleeding, perforation, obstruction, and/or aggression then the tumor indicates advanced-stage of colon cancer. Based on the clinical and microscopy findings, the Type of Chemotherapy is decided.
