Accidents do not just happen—they are caused, and the key to accident investigation is to find the causes. The main reason for investigating any industrial incident is to prevent a recurrence of that accident. Incident / Accident Investigations are essential to risk and safety management. Learning from disasters is critical to preventing future accidents and incidents.
