The key to the phaco flip technique is using the fluid wave from hydro-dissection to gently prolapse part of the nucleus out of the capsular bag. Phaco flip works very well for the softer lens nuclei (1 to 2+NS) that we see in younger patients (50s and 60s). There is no force in this technique and a previous video shows that the fluidic pressure is quite low. Once this partial nucleus prolapse is completed, the 27g blunt cannula can be used to further manipulate the cataract so that it does not return to the capsular bag. Remember that this technique requires a good 5 to 5.5 mm diameter capsulorhexis, so if you have made too small of an anterior capsular opening or if the capsulorhexis is irregular, then you should avoid this method.
