How can one prevent a future unwanted pregnancy? | Unwanted Pregnancy - Dr. Punitha Rangaraj

If you are sexually active and not planning pregnancy it is better to use a contraceptive than going for an abortion. One can choose from your requirement and acceptability from various methods of contraception that are available. But all the contraceptives will not give 100 % protection. First one is timing of the intercourse. If you have a regular periods then you ovulate in the middle of the periods. Avoid sex in the middle 10 days, the first and last 10 days are safe. But if the periods is not regular this method gives a very high failure rate. The second method is to time your ovulation. There are ovulation kits available. Start planning your ovulation from the eighth day of your cycle. Just put a drop of urine to the ovulation kit and see whether it is positive. The day it becomes positive that means you are going to ovulate in another 24 to 36 hours. So you should prevent having sex from that day to another 5 to 6 days. If not you can go for a barrier method called diaphragm or a female condom. It is a rubber disk with a flexible rim which can be fitted into the vagina just before having an intercourse. It prevents the semen from entering the uterus and thus fertilization doesn’t take place. You have to keep it for a minimum of 6 hours or maximum of 24 hours then you remove it and discard it. Failure rate can happen if not fixed properly and if wrong size is used. Next is oral contraceptive pills which is a combination of oestrogen and progesterone when given cyclically prevents ovulation. When you are taking this tablet you have to be very regular. Suppose you miss the tablet for a day or two, you may get pregnant and have a spotting in between called the break through bleeding. Some of them will experience dizziness, nausea, vomiting, mood swings or a weight gain. Next method is long acting injectables which are a depo injections given deep intra muscular into the buttocks and can be taken once in three months. Mode of action is similar to oral contraceptives. You may have irregular spotting throughout the month or you may not get the periods at all. Once you stop the injection your menstrual cycle reverts back to normal. Next method is medicated implants that can be implanted by making a small incision in the upper arm. This is a long acting contraceptive which acts by slow release of the hormone and preventing ovulation for 3 to 4 years. Other methods could be intra uterine contraceptive devices called IUDs and very commonly called as loop. This can be used for people who are going for a long term contraception or for women who want to space their pregnancy. It is an outpatient procedure. It releases copper or medicines continuously and causes chemical changes to the egg or sperm. Last is a sterilization method which is a surgical method and advised only for patients who have finished their family way. There are two type of surgeries we do. One is the convectional tubectomy where a small incision is made in the abdomen, the tubes are taken out, tied, put back and sutured. In laparoscopy we make two small holes on the abdomen, the scope goes inside and we put some breaks on both sides of the fallopian tubes and blocks it. There are two ways of male contraception. Condoms act as a barrier from the semen to the vagina. It not only prevents pregnancy but also sexually transmitted diseases. Second method is vasectomy, a surgery where vas deferens is blocked and prevents the semen from entering into the seminal fluid. It doesn’t interfere with the proper sexual life. Last one is emergency contraception which is used after having an unprotected sex. There are tablets which are available which can be taken any time between 24 hours to 5 days, which can be taken as a single dose or at the interval of 12 hours. By this it delays ovulation or brings some changes in the sperm so that it prevents fertilization. The earlier you take the tablet the success rate would be better. IUDs can also be used as an emergency contraception.
