Dr Tom Harcourt Brown joins long-time friend, Hannah Capon to talk about lumbosacral disease. During an extended chat they manage to cover: – What lumbosacral disease is with a brief review of the anatomy of the area. The outcome is agreeing it is more of a syndrome where a number of overlapping/ similar clinical signs occur which don't have a definite cause. – The clinical signs of lumbosacral disease are varied because there is a mixed pathology of causes from muscle weakness to significant neuropathic pain. – That there are many variations of the pathology within the term lumbosacral disease from cauda equina compression to nerve root pinching. There are also other sources of discomfort such as articular facet pain and muscle pain. – Tom’s view is that true lumbosacral disease requiring surgical intervention is actually less common, and mild to moderate clinical signs often lead to conservative management. – That often the diagnostic investigations don't lead to a decisive action but will lead to ruling out other possible pathologies and allow the owner to approach the case conservatively initially and for a review to decide whether a more invasive approach is appropriate. – That there are different surgeries being performed by different clinicians and at present none are shown to be advantageous over the others. – That there are also alternative approaches such as epidural steroids and perineural steroids with local anaesthetic, the latter often offering a diagnostic plus treatment approach to the case. – Hannah advises on conservative management with weight control being at the forefront. Tom Harcourt-Brown graduated from the University of Cambridge in 2002 and worked in a general small animal practice on the outskirts of London before returning to his home town of Harrogate in Yorkshire to work in his parents' practice between 2003 and 2007. During this time he studied for his RCVS certificate in diagnostic imaging, which he was awarded in 2007. Following his employment in Harrogate, he took up a post as a senior clinical training scholar in neurology and neurosurgery at the Queens Veterinary School Hospital in the University of Cambridge. He passed the examination for the European College of Veterinary Neurologists in September 2010. Tom has been at Langford Vets since 2010, initially running the neurology referral department on his own but now part of a growing team of senior neurologists, residents in training and one full time neurology nurse catering to one of the busiest referral departments in the UK. He is primarily responsible for seeing and supervising cases referred to the hospital but also spends time teaching the residents and final year students on rotation, as well as being one of the examiners for the European Diploma in Veterinary Neurology. In 2020 Tom was awarded a fellowship of the Royal College of Veterinary Surgeons for meritorious contributions to clinical practice.
