Mark Zafereo, Eyal Robenshtok, Margaret Brandwein-Weber, Jill Langer, Bruce Culliney and Dana Hartl. 0:00 Webinar 0:28 Get the Podcast! 0:48 Introductions 2:15 Case 1 A 32-year-old woman with a history of aplastic anemia and graft-versus-host disease developed a palpable thyroid nodule despite normal thyroid function. Neck ultrasound identified a suspicious 1.8 cm right thyroid nodule, classified as TIRADS V. Initially, she declined fine-needle aspiration (FNA) but later agreed after the nodule grew to 2.2 cm. Results indicated atypia of undetermined significance (AUS) with a ROS1 fusion, leading to a right hemithyroidectomy, which confirmed invasive PTC. Post-surgery, metastatic PTC was found in neck nodes. During a subsequent pregnancy, nodal disease progressed, and she underwent a complete thyroidectomy and bilateral neck dissection. Postpartum, her thyroglobulin remained elevated, and she plans to undergo radioactive iodine therapy after finishing breastfeeding. 30:56 Case 2 A 36-year-old man with type II diabetes, sleep apnea, and hypertension was diagnosed with multifocal papillary thyroid cancer after a neck ultrasound and FNA revealed a 4.2 cm nodule on the left side. He underwent a total thyroidectomy, followed by radioactive iodine therapy, but developed biochemical recurrence and local disease, including metastatic PTC. Despite multiple surgeries and recurrences, he preferred surgical management over external radiation. Over several years, he experienced rapid recurrences in the neck and surrounding tissues, leading to multiple neck dissections. Now 41, he has radioiodine-refractory disease and continues close surveillance.
